Appeal forms and all supporting documentation must be submitted at least four weeks prior to the end of a term.
- 2026-2027 Forms (includes Fall 2026, Winter 2026, J-term 2027, Spring 2027, and Summer 2027)
- 2025-2026 Forms (includes Fall 2025, Winter 2025, J-term 2026, Spring 2026, and Summer 2026)
Students should login to our secure server using their Loyola username and password in order to submit Financial Aid documents. If parents or others would like to submit documents on a student’s behalf, the student may grant them access through the secure upload link.
Documents can also be dropped off in-person at the HUB desk in the Sullivan Center, faxed to 773-508-3397, or mailed to:
