IMMUNIZATION REQUIREMENTS FOR VISITING PHYSICIANS-In-TRAINING

All visiting student applicants must comply with our immunization requirements, documentation of which will be reviewed by the Health Sciences Campus Wellness Center.  Please note that you are required to show titer results to indicate proof of immunity. Our immunization requirements are below:

Loyola University Chicago Stritch School of Medicine requires students to show proof of surveillance for tuberculosis infection within 12 months of their scheduled clinical rotation and proof of immunization against Measles, Mumps, Rubella, Varicella, Hepatitis B, and Tetanus, Diphtheria & Pertussis (TDAP vaccine). You also must include lab report PDF's with numeric titer results  to prove your Measles, Mumps, Rubella, Varicella, and Hepatitis B immunity. Proof of annual influenza immunization required for rotations between November 1 and April 30.

Rotations will be cancelled for physicians-in-training who have not been vaccinated or who do not submit the required documentation in VSLO.

FLU VACCINE REQUIREMENT DETAILS:

All students, physicians and staff at Loyola University Medical Center are required to be vaccinated for the seasonal flu. It is mandatory for all visiting students participating in electives at Loyola (and affiliated sites) to show proof of seasonal flu vaccination, beginning with the November elective rotations.

You will not be allowed to participate in your scheduled elective unless you are vaccinated and will be required to show proof of your seasonal flu vaccine before you can begin your elective. Please provide the documentation of your vaccination to your elective coordinator upon arrival at your elective service.

Acceptable documentation includes a signed statement from the vaccine provider giving the date, type of vaccine, name and address of the provider, the manufacturer, lot number, and dose. If you have had your flu vaccine this year but have misplaced your documentation, your elective coordinator will give you a form to fill out attesting to this.
