This form is to be used for anonymously reporting a suspected disclosure of sensitive personal information. 

Please provide as much detail as possible regarding the circumstances in which the sensitive data was exposed, disclosed, or otherwise improperly used. Any information provided will only be viewed by the Loyola Information Security & Compliance team.
